This position is part of a team which provides Assertive Community Treatment to individuals with severe and persistent behavioral health disorders who have been discharged from multiple or extended stays in public hospitals or who are difficult to engage in treatment. The Assertive Community Treatment (ACT) program utilize an interdisciplinary approach to provide intensive, integrated and rehabilitative crisis treatment and community support interventions/service that are available 24 hours per day, 7 days per week. Consequently, all team members must be available at any time that assigned clients need assistance.
 
The Behavioral Health Nurse, ACT, is a registered nurse who provides nursing assessments and medication management for adults with behavioral health or substance abuse disorders. Services are designed to treat the acutely-distressed consumer in the community with no delay, preventing hospitalization, incarceration or other placements.
The ACT team nurse performs a biopsychosocial and physical nursing assessment and uses the information to develop a preliminary nursing plan. In conjunction with the psychiatrist, administers any initial and ongoing medications prescribed. Works with the clients and other members of the team to develop and implement a comprehensive individual service plan that includes nursing care and medication management. Evaluates client care and progress to ensure immediate attention to changing medical treatment needs.
 
Provides on-call, after hours services on a rotational schedule
Provides to the client and family members as needed, information about management of medication, complications and side effects. May teach in an individual or group setting behavioral symptoms management techniques to alleviate and manage symptoms not reduced by medication and/or adaptation/coping techniques for internal and external stresses.
 
In collaboration with the psychiatrist, coordinates schedules and administers medical assessments of clients' physical health. Makes appropriate referrals to community physician for further assessment and coordinates psychiatric and medical treatment.
Maintain medical records and complies with third party payer requirements for documentation and authorizations. Provide accurate and timely documentation of charts in a computerized record system.
Shift: Mon - Fri (1st Shift) - Includes on-call duties
Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience.
