 Public Health Registered Nurse 2 or 3

Children's Medical Services (CMS) 

District 4 Public Health

           LaGrange, Georgia 30240           

        

Job Description:  Under general supervision, provides professional nursing care coordination in a specialized Public Health program, per policy and procedure manual. Conducts face-to-face home visits within designated counties. Assesses the client's condition, develops plans of care, works in clinics, refers children with chronic illnesses to the appropriate services, educates clients/parents and refers clients to agencies that can assist in meeting social and/or health needs. Records pertinent observations and responses to treatment. Receives referrals, collects data, assesses the data and determines services needed. Note:This position requires travel to all 12 District 4 counties. Travel reimbursement will be based on current State of Georgia Travel regulations.

 

Minimum Qualifications:  

Registered Nurse Level 2

Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in Nursing from an accredited college or university AND One year PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Nurse license AND CPR certification. Target hire-in salary $47,025*

 

Registered Nurse Level 3

Bachelor's degree in Nursing from an accredited college or university AND One year of PHN experience and Current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in Nursing from an accredited college or university AND Two years of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse License AND CPR certification. Target hire-in salary $51,727.50* 


*Salary offer will be based on Education, Experience & Department Budget
