Behavioral Health Assessment Clinician
The Behavioral Health Assessment Clinician provides
comprehensive mental health and addictive disease assessments for consumers
seeking treatment services.
ü Utilizes the interview
information to complete a comprehensive assessment of needs, level of care, and
initial treatment recommendations
ü Provides initial program
orientation to persons presenting for treatment of mental health and/or
addictive diseases.
ü Assesses client’s
treatment needs and assists him/her to develop appropriate individual treatment
goals.
ü Writes clear, concise,
complete, grammatically correct, and timely progress notes that relate directly
to the treatment outcomes described in the service plan (24 hour
documentation).
ü
May serve as case manager to facilitate and ensure communication,
linkage and continuity of care between consumers and other treatment team
members.
ü Educates
families/significant others on mental health diagnoses, addiction and recovery
process.
ü Provides
crisis intervention services to existing and walk in clients, as needed.
ü Establishes
linkages and coordination of services with community agencies
ü Executes
legal forms required to refer the consumer to emergency receiving facilities.
ü Applicant must be skilled in the
diagnostic process of diagnosing with DSM-5 and treatment planning while
processing knowledge and skill of using various therapeutic evidence based
interventions.
ü Follows
all Medicaid, state, insurer, and other regulations related to the credentials
required to provide authorized services.
